Prob. w/ "visible if recently used" items

The Alt+key should still access a hidden menu item. The accelerator still works, but you just get a loud windows 'Ding!' and no action if the menu item you try to shortcut to is hidden.  Say "&File/&Save"  is currently hidden because it hasn't been recently used.  If your habbit is to press ALT+F, then S, you won't always get to save.  Assuming the menu item does not have a shortcut like Ctrl+S, the best you can do is ALT+F, then UP, then DOWN, then S.  This is quite annoying to power users who almost never use a mouse for accessing the menus.
