Another quick question.

We'd like to offer our users alternate key-bindings (keyboard accelerators) that are used by a competing product(s). Is it possible to switch key-binding for commands/menu-items on the fly at run-time?

Ideally, we'd like to read the key-bindings from a flat file. What is the best way to do this?

Yes you can.
See CXTPShortcutManager class in help.

f.e
pCommandBars->GetShortcutManager()->UpdateAcellTable(...);
