Can't get rid of an old icon in the toolbar!

It's really strange, but I just can't seem to find a way to get rid of an old icon in the toolbar - I've replaced it with a new graphic, but the new graphic doesn't show up...

I've tried killing the registry for the whole application, cleaning solution/rebuilding, changing the toolbar graphic filename, changing the ID for the button - nothing

All I want is for the new graphic to show up in place of the old one - what am I missing?

I don't know what happened, but my release builds are ok, only the debug ones have the weirdly screwed up graphics. I'll see if I can discover any differences.
