How to programmingly add toobar buttons?

I have a program that allow user to add tools to a toolbar. I want to convert it to use XT toolkit. But If I load toolbar's state on startup, the new toolbar buttons added will not appear. How do I solve it? I can not always reset toolbar's configuration after user adds a new tool.

Any suggestion is appreciated.

May be it was added with Temporary flag? Give us more code - How you Load/Save, how you Add custom buttons....
